Provides that on and after January 1, 2017, an insurer that issues health insurance in the State of Illinois must provide each member covered under a policy or plan with a benefits identification card. Provides that the benefits identification card shall include the member's name, account tracking number, copayment and coinsurance amounts, and contact information for the insurer.
