Requires the Department of Public Health to establish a 3-year pilot program that provides diabetes testing and education on diabetes management in Cook County, DeKalb County, and Rock Island County from funds appropriated for that purpose. Provides that the Department shall designate a single community-based, not-for-profit provider of healthy living services to manage and administer the program; and that the designated provider shall receive reimbursement under the medical assistance program for the diabetes testing and educational services provided under the pilot program.
