| HOUSE RESOLUTION
|
| 2 |
| WHEREAS, Several State-operated facilities and State |
| 3 |
| psychiatric hospitals that serve adults with developmental |
| 4 |
| disabilities and mental illness have closed in recent years; |
| 5 |
| and
|
| 6 |
| WHEREAS, It is critical that former residents of these |
| 7 |
| facilities do not lose vital health care services as they |
| 8 |
| transition to a new service provider, whether it be a |
| 9 |
| community-based agency, another State-operated facility or |
| 10 |
| State psychiatric hospital, or home care; therefore, be it
|
| 11 |
| RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE |
| 12 |
| NINETY-SIXTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that |
| 13 |
| the Department of Human Services shall file a report detailing |
| 14 |
| the number of individuals that have been transferred or |
| 15 |
| transitioned out of a State-operated facility or State |
| 16 |
| psychiatric hospital due to facility closures; where these |
| 17 |
| individuals were transferred or transitioned to; how these |
| 18 |
| individuals are meeting their health care needs following |
| 19 |
| transfer or transition; whether these individuals have |
| 20 |
| experienced a break in health care service and, if applicable, |
| 21 |
| the length of time these individuals went without health care |
| 22 |
| service; and how these individuals are meeting their health |
| 23 |
| care needs one year later; and be it further
