| AMENDMENT TO SENATE BILL 715

| AMENDMENT NO. ______. Amend Senate Bill 715 on page 2, by |

| inserting immediately below line 7 the following:

| "Section 10. Eligibility. All students in the school under |
| 5 |
| the age of 18 are eligible for services if they have obtained |
| 6 |
| written parental consent or if they are otherwise permitted |
| 7 |
| under Illinois law to consent on their own behalf to such care. |
| 8 |
| All students 18 years of age or older are eligible for the |
| 9 |
| services.

| 10 |
| Section 15. Consent. The school health center shall provide |
| 11 |
| a list of the health care services available. The form shall |
| 12 |
| enumerate the provided services using either a check off or |
| 13 |
| other means. The consent form shall state that a parent, legal |
| 14 |
| guardian, or student who is permitted under Illinois law to |
| 15 |
| consent on his or her own behalf has a right to refuse any |
| 16 |
| health care services."; and
