| AN ACT concerning criminal law.

| Be it enacted by the People of the State of Illinois, |
| 3 |
| represented in the General Assembly:

| Section 5. The Unified Code of Corrections is amended by |
| 5 |
| changing Section 3-5-2 as follows:

| persons and what grievances, if any, are
made in each of its |

| institutions, facilities and programs. The record shall

| contain the name of the persons involved, the time, date, place |
| 13 |
| and purpose
of the procedure, the decision and basis therefor, |
| 14 |
| and any review of the
decision made. |
