| HOUSE RESOLUTION
|
| 2 |
| WHEREAS, Bishop Horace E. Smith, M.D., became pastor of the |
| 3 |
| Apostolic Faith Church on March 28, 1980; and
|
| 4 |
| WHEREAS, Dr. Smith was educated in Chicago, having received |
| 5 |
| a B.S. from Chicago State University, an M.D. from the |
| 6 |
| University of Illinois Medical Center, Pediatric Residency at |
| 7 |
| the University of Illinois Hospital, and Completion of Clinical |
| 8 |
| Fellowship at Children's Memorial Hospital at Northwestern |
| 9 |
| University; and
|
| 10 |
| WHEREAS, Dr. Smith is recognized not only for his medical |
| 11 |
| expertise, but also his dedication to public service, including |
| 12 |
| his service on the Board of the historic Wabash YMCA, the Board |
| 13 |
| of the Family Institute at Northwestern University, and as a |
| 14 |
| member of the University of Chicago Cancer Research |
| 15 |
| Center-Community Health Advisory Board; and
|
| 16 |
| WHEREAS, Dr. Smith was elevated to Bishop and Diocesan in |
| 17 |
| the Pentecostal Assemblies of the World in 1998; and
|
| 18 |
| WHEREAS, Dr. Smith is partnered in life and ministry with |
| 19 |
| his wife Susan Davenport Smith, with whom he has three |
| 20 |
| daughters; and
|
| 21 |
| WHEREAS, The Apostolic Faith Church celebrates the 26th |
| 22 |
| Pastoral Anniversary of Dr. Smith March 24 through March 26, |
| 23 |
| 2006; therefore, be it
|

| RESOLVED, BY THE HOUSE OF REPRESENTATIVES OF THE |
| 25 |
| NINETY-FOURTH GENERAL ASSEMBLY OF THE STATE OF ILLINOIS, that |
| 26 |
| we congratulate Bishop Horace E. Smith, M.D., on his Pastoral |
| 27 |
| Anniversary and thank him for his many years of dedicated |
| 28 |
| service to our community; and be it further
