| AMENDMENT TO HOUSE BILL 1350

| AMENDMENT NO. ______. Amend House Bill 1350 on page 2, by |
| 3 |
| replacing lines 26 and 27 with the following: |
| 4 |
| "copy of the medical certification, with information |
| 5 |
| documenting that the death was due to a vehicle backing over |
| 6 |
| the child or that the death was caused by a power window of a |
| 7 |
| vehicle, to the Department of Children and Family Services. The |
| 8 |
| Department of Children and Family Services shall (i) collect |
| 9 |
| this information for use by Child Death Review Teams and (ii) |
| 10 |
| compile and maintain this information as part of its Annual |
| 11 |
| Child Death Review Team Report to the General Assembly."; and

| 12 |
| on page 3, by replacing lines 2 through 4 with the following: |
| 13 |
| "the power window of a motor vehicle, the coroner or medical |
| 14 |
| examiner must send a copy of the medical certification, with |
| 15 |
| information documenting that the death was due to a vehicle |
| 16 |
| backing over the child or that the death was caused by a power |
| 17 |
| window of a vehicle, to the Department of Children and Family |
| 18 |
| Services. The Department of Children and Family Services shall |
| 19 |
| (i) collect this information for use by Child Death Review |
| 20 |
| Teams and (ii) compile and maintain this information as part of |
| 21 |
| its Annual Child Death Review Team Report to the General |
| 22 |
| Assembly.".
