Requires the Department of Human Services to establish and administer a family case management program to help low-income families obtain available health and human services needed for healthy growth and development, including but not limited to immunizations, lead screenings, nutritional support, and other specialized services for families with additional challenges and needs. Provides that under the program, case management shall involve individualized assessment of needs, planning of services, referral, monitoring, and advocacy to assist a client in gaining access to appropriate services. Provides that under the program, case management shall be an active and collaborative process involving a qualified case manager, the client, the client's family, and service providers in the community. Creates the Maternal and Child Health Advisory Board to advise the Department in implementing the Act. Amends the Infant Mortality Reduction Act and the Prenatal and Newborn Care Act. Abolishes the Infant Mortality Reduction Advisory Board, and provides that the Department shall consult with the Maternal and Child Health Advisory Board (instead of the Infant Mortality Reduction Advisory Board) in administering the Infant Mortality Reduction Act and the Prenatal and Newborn Care Act. Effective immediately.House Amendment No. 1 Deletes reference to:410 ILCS 220/7Adds reference to:410 ILCS 220/Act rep.Replaces everything after the enacting clause with provisions substantially similar to those of House Bill 2374, but with changes that include the following: (1) changes the short title of the new Act to the Illinois Family Case Management Act (instead of the Family Case Management Act); (2) adds a definition of "Targeted Intensive Case Management"; (3) adds, as a purpose of the Family Case Management Program, reducing the incidence of infant mortality, very low birthweight infants, and low birthweight infants; (4) provides for the Governor's appointment of 17 (instead of 12) members of the Maternal and Child Health Advisory Board, and changes the qualifications of those members; and (5) repeals (instead of amending) the Infant Mortality Reduction Act. Effective immediately.House Amendment No. 3 Replaces everything after the enacting clause with provisions substantially similar to those of House Bill 2374 as amended by House Amendment No. 1, but with changes that include the following: (1) changes the definition of "eligible participant" and deletes the definition of "eligible organization"; (2) provides for the Governor's appointment of 20 (instead of 17) members of the Maternal and Child Health Advisory Board, including (i) 5 representatives, one each from certified local health departments within the 5 counties with the largest number of family case management enrollees (instead of representatives based on recommendations by the IAPHA), (ii) 5 representatives from certified local health departments outside the Chicago metropolitan and collar counties areas that shall include a balance of urban and rural health departments (instead of representatives based on recommendations by the NIPHC), and (iii) 5 individuals representing community-based providers of family case management services within Cook County that are not certified local health departments (instead of individuals based on recommendations by the CFCMC); and (3) requires the Board to report by January 1 (instead of April 15) of each year.
