AMENDMENT TO HOUSE BILL 4762 

2    AMENDMENT NO. ______. Amend House Bill 4762 on page 22, by 
3replacing lines 24 through 26 with the following:
4        "(1) 5 years or more have lapsed since the later of: 
5    (A) entry of a final disciplinary order against the 
6    applicant or licensee or (B) restoration of the license 
7    after the order;"; and
 
8on page 23, by replacing line 9 with the following:
9"this subsection (e) do not include violations based on:"; and
 
10on page 23, line 10, by replacing "for health care workers as" 
11with "of health care workers for the offenses"; and
 
12on page 23, line 19, by replacing "for massage therapists" 
13with "of massage therapists for the offenses"; and
 
14on page 24, by replacing line 6 with the following:
