| 5 | | on page 1, lines 15 and 22, by replacing "mother" with "patient |
| 6 | | mother"; and |
| 7 | | on page 2, line 10, after "develop", by inserting "language |
| 8 | | on"; and |
| 9 | | on page 2, by replacing lines 12 through 19 with "gestational |
| 10 | | parent. This section of language shall be known as a "Liam's |
| 11 | | Law notice". The "Liam's Law notice" shall be available in |
| 12 | | both English and Spanish."; and |
| 13 | | on page 3, line 2, by replacing "mother" with "patient |
| 14 | | mother"; and |

| 1 | | on page 5, by replacing lines 2 through 12 with the following: |
| 2 | | "(b) After each fetal death that occurs in this State |
| 3 | | after a gestation period of at least 20 completed weeks, the |
| 4 | | State Registrar of Vital Records shall, only upon request by a |
| 5 | | parent named on the fetal death certificate, prepare and issue |
| 6 | | a certificate of birth resulting in stillbirth. After each |
| 7 | | fetal death that occurs in this State after a gestation period |
| 8 | | of at least 26 completed weeks, the person who files a fetal |
| 9 | | death certificate in connection with that death as required |
| 10 | | under Section 20 shall, only upon request by the woman who |
| 11 | | delivered the stillborn fetus, also prepare a certificate of |
| 12 | | stillbirth. The person shall prepare the certificate on the |
| 13 | | form prescribed and furnished by the State Registrar and in |
| 14 | | accordance with the rules adopted by the State Registrar."; |

| 18 | | on page 6, lines 18 and 19, by replacing "upon becoming law" |
