| 2 | | WHEREAS, The members of the Illinois Senate are saddened to |
| 3 | | learn of the death of Specialist Henry Jarrett Mayfield Jr. of |
| 4 | | Hazel Crest, who passed away while on duty in Kenya, Africa on |
| 5 | | January 5, 2020; and
|
| 6 | | WHEREAS, SPC Mayfield Jr. was born to Henry Mayfield Sr. |
| 7 | | and Crystal Davis on October 14, 1996; he attended Lionel |
| 8 | | Hampton Elementary, William J. Bogan High School, and later |
| 9 | | graduated from Hillcrest High School in 2014; he then attended |
| 10 | | Northern Illinois University, where he majored in business; and
|
| 11 | | WHEREAS, SPC Mayfield Jr. began working as an Aircraft |
| 12 | | Fueler at Aircraft Service International Group (ASIG) in 2016; |
| 13 | | and
|
| 14 | | WHEREAS, SPC Mayfield Jr. enlisted in the U.S. Army on |
| 15 | | August 21, 2017; he attended Basic Combat Training at Fort |
| 16 | | Jackson, South Carolina and Advanced Individual Training at |
| 17 | | Fort Lee, Virginia; he was awarded the MOS of 91C Utilities |
| 18 | | Equipment Repairer; he was assigned to the 1st Battalion, 58th |
| 19 | | Aviation Regiment at Fort Rucker, Alabama; he was deployed with |
| 20 | | the 1st Battalion, 58th Aviation Regiment to Kenya, Africa in |
| 21 | | support of Operation Enduring Freedom-Horn of Africa in October |
| 22 | | 2019; his awards and decorations included the Combat Action |
|

| 1 | | Badge (posthumous), the Bronze Star Medal (posthumous), the |
| 2 | | Purple Heart (posthumous), the Army Achievement Medal, the |
| 3 | | National Defense Service Medal, the Global War on Terrorism |
| 4 | | Expeditionary Medal, the Global War on Terrorism Service Medal, |
| 5 | | the Overseas Ribbon, and the Army Service Ribbon; and
|
| 6 | | WHEREAS, SPC Mayfield Jr. committed to his faith at an |
| 7 | | early age and was baptized by Reverend Marion Johnson Sr. of |
| 8 | | Come Alive Ministry of Faith Church; he was affectionately |
| 9 | | known as "Boo-Boo" to his family and enjoyed spending quality |
| 10 | | time with his siblings and cousins; he is remembered for |
| 11 | | bringing joy to anyone who met him; and
|
| 12 | | WHEREAS, SPC Mayfield Jr. was preceded in death by his |
| 13 | | great grandmother, Jacqueline Overton; and his grandparents, |
| 14 | | Ronnie E. Mayfield Sr., Stephanie Mayfield, and Levola Reed; |
| 15 | | and
|
| 16 | | WHEREAS, SPC Mayfield Jr. is survived by his parents, Henry |
| 17 | | Mayfield Sr. and Carmoneta Horton-Mayfield; his biological |
| 18 | | mother, Crystal Davis; his fiancée, Ce ara Hughes; his |
| 19 | | grandmother, Annette Horton; his grandfather, Michael |
| 20 | | Thompson; his brothers, Ronald, Tyron, Michael, and Nicholas; |
| 21 | | his sisters, Danielle (Cornelia), Ciara, Taliyah, and |
| 22 | | Tyshauna; his uncles, Ronnie Jr. (Sonya), Ricky (Detrich), |
| 23 | | Anthony (Mattie), and Melvin; his aunts, Elizabeth, Chaquitta, |

| 3 | | RESOLVED, BY THE SENATE OF THE ONE HUNDRED FIRST GENERAL |
| 4 | | ASSEMBLY OF THE STATE OF ILLINOIS, that we mourn the passing of |
| 5 | | Specialist Henry Jarrett Mayfield Jr. and extend our sincere |
| 6 | | condolences to his family, friends, and all who knew and loved |
| 7 | | him; and be it further

| 8 | | RESOLVED, That a suitable copy of this resolution be |
| 9 | | presented to the family of SPC Mayfield Jr. as an expression of |
