<reference>720 ILCS 5/16-0.1</reference><aliasreference></aliasreference><SynopsisText>     Amends the Criminal Code of 2012. Adds, to the definition of "personal identifying information" for purposes of identity theft: (1) any information regarding an individual's medical history, mental or physical condition, or medical treatment or diagnosis by a health care professional and (2) a person's health insurance policy number or subscriber identification number, any unique identifier used by a health insurer to identify a person, or any information in an individual's application and claims history, including, but not limited to, appeals history.</SynopsisText></synopsis>
