| 2 | | WHEREAS, The members of the Illinois Senate are saddened to |
| 3 | | learn of the death of Daniel R. Patch Sr. of Lindenhurst, who |
| 4 | | passed away on April 19, 2017; and
|
| 5 | | WHEREAS, Daniel Patch was born to Oliver and Helen (Daly) |
| 6 | | Patch on March 15, 1944; he graduated from St. George High |
| 7 | | School in 1962 and from the Worsham College of Mortuary Science |
| 8 | | in 1966; he served in the United States Army during the Vietnam |
| 9 | | War from 1967 to 1969; and |
| 10 | | WHEREAS, Daniel Patch was a licensed funeral director for |
| 11 | | over 50 years; he and his son, Daniel Jr., owned and operated |
| 12 | | the Peterson and Patch Funeral Home and The Memorial Chapel of |
| 13 | | Waukegan; and
|
| 14 | | WHEREAS, Daniel Patch was a member of the National Funeral |
| 15 | | Directors Association, the Illinois Funeral Directors |
| 16 | | Association, and the Northeastern Illinois Funeral Directors |
| 17 | | Association, where he served as president from 1986 to 1988; |
| 18 | | and
|
| 19 | | WHEREAS, Daniel Patch was a member of Homer Dahringer |
| 20 | | American Legion Post 281, a past member of the Waukegan |
| 21 | | Jaycees, a past president and member of the Waukegan Lions |
|

| 1 | | Club, the Waukegan Moose Lodge, the Knights of Columbus, and |
| 2 | | the Lindenhurst Lakes Commission; and
|
| 3 | | WHEREAS, Daniel Patch is survived by his wife of 50 years, |
| 4 | | Theresa (Laffredi); his children, Dana (John) Iskalis, Dawn |
| 5 | | (Brett) Stickels, and Daniel R. Jr. (Michelle) Patch; his |
| 6 | | grandchildren, Thomas, Taylor, Sydney Iskalis, Connor, Boden |
| 7 | | Stickels, Oliver, and Cecelia Patch; his sister, Virginia |
| 8 | | Baker; his sisters-in-law, Linda (Jim) Onan, and Carol (Larry) |
| 9 | | Jackson; numerous nieces, nephews, and extended family; and his |
| 10 | | German Shepherd, Tuffy; therefore, be it
|
| 11 | | RESOLVED, BY THE SENATE OF THE ONE HUNDREDTH GENERAL |
| 12 | | ASSEMBLY OF THE STATE OF ILLINOIS, that we mourn the passing of |
| 13 | | Daniel R. Patch Sr., and extend our sincere condolences to his |
| 14 | | family, friends, and all who knew and loved him; and be it |
| 15 | | further

| 16 | | RESOLVED, That a suitable copy of this resolution be |
| 17 | | presented to the family of Daniel Patch as an expression of our |
| 18 | | deepest sympathy.
