| 2 | | Be it enacted by the People of the State of Illinois, |
| 3 | | represented in the General Assembly:

| 4 | | Section 5. The Illinois Dental Practice Act is amended by |
| 5 | | changing Section 8.05 as follows:
|

| 13 | | records pertaining to the license. As soon as practical, the |
| 14 | | Department shall assign a customer's identification number to |
| 15 | | each applicant for a license. |
| 16 | | Every application for a renewal or restored license shall |
| 17 | | require the applicant's customer identification number.
